I can't forget to include the other member of our family on my blog. Justin and I got our chocolate lab Augie less than a month after we got married. For 5.5 years, she was our very spoiled child. She is still a quite spoiled house dog who sleeps in our bed with us. When we brought Aria home, Augie didn't quite know what to think. She pretty much just laid in whatever room we were in and didn't move. She soon learned that small, crying baby was a part of our family and returned to normal. She definitely loves the poopy diapers. Augie is so smart that now when I put Aria down for a nap, Augie immediately runs to the back door in hopes of going outside to play. So, the following video is what we do while Aria sleeps. Sometimes, I don't have the best arm and Augie has to improvise!

The Results of Faith, Hope, and Perseverance: Success!
Obviously, since we have Aria and by my title, we did have success the second time around with in vitro. However, we didn't get through it without some discouragement. As I mentioned in my previous entry, the doctors decided to try the drugs a bit differently this time. The cycle was shorter and I had higher doses of drugs. I didn't really notice a difference in how I physically responded, but my body did. The first time we went through the cycle, I created many eggs and they grew just how they were supposed to. This time, things didn't look as promising. When we went for the ultrasounds and blood tests, I just wasn't responding to the drugs like they hoped. Instead of making even more eggs this time, there were fewer. I feared we tried again too soon. I thought my body was just confused with all the different drugs over the last 3 months. The doctors said that wasn't true, though. When they originally mapped out the cycle for us, we were going to be doing a retrieval in the last few days of April or the first couple days of May. They just kept pushing it back and giving me more drugs in hopes of getting more eggs. Finally, we could wait no longer and we had to go in for the egg retrieval again on May 7. I won't describe the process again, but we experienced another "these people want Justin and Julie Pegump to have a baby" moment. On the ultrasounds, it appeared we may only get 8-10 eggs this time to start with. Well, the main doctor we dealt with didn't perform my egg retrieval procedure this time, at least at the start. We learned after the procedure that he came in part way through and finished the egg retrieval to make sure he got every egg. They got 14 eggs! We were shocked! It turned out our cycle was very similar to the first one, but with very different results! There were only 8 eggs that fertilized and made it past the first day. Of those 8, there was 1 really great one and the rest not so great. Again, that 1 really great one was the one that Reprogenetics told us was either normal or balanced! However, this time when we went for the transfer on May 12, the embryo was in the blastocyst stage, which is much further along and much healthier than we had the first time. I've included pictures for you to see Aria in her early stages! I know this looks like a blob to everyone, but notice how there are 2 separate clusters? One of those clusters turns into the baby and one turns into the placenta. I didn't include the pictures of the other embryos we had that had already died, but there was not 2 separate clusters on any of those. Not to make the men out there squirmy, but having Aria transferred into my uterus feels similar to the yearly exam we get ladies. And, I'll tell you, I've never had so much fun at one of those as I did on May 12. We had a team of 2 female doctors, the female embryologist, and a female nurse. They were all on a roll that day, cracking jokes and encouraging us so much! They asked what I did and I told them about teaching TAG students and I got to hear some great stories from those very intelligent doctors about their school days and how bored they often were. I just knew by the positive vibe they gave off in that room that this in vitro cycle was going to work. However, we were much more cautious this time. We didn't really celebrate that much because we could still feel the pain of February 17, less than 3 months ago at that point. So, we waited on pins and needles for May 22 when we could hear the news of pregnant or not. In those 10 days, I wanted to feel confident because I physically felt different. But, I thought my mind could just be playing tricks. Well, May 22 came. On that day we went to the University in the morning to do the blood test. We decided we would get together after work and call the phone line to hear the results together. It was a Friday and I could leave school at 2:45. Justin took part of the afternoon off to be home about 3 with me. He beat me home. As I was driving home I received a call from a University number. I didn't want to answer it until I had Justin with me and especially not while I was driving because the emotions probably would cause a wreck. So, the person left a message. When I got home I told Justin about the call and we decided to listen to the message. On the message, it was a nurse from the University telling me she really needed me to call her back ASAP so she could get that prescription to me before the long weekend. We knew that if I was not pregnant, I wouldn't need a prescription. So, then we called the phone line to be sure and heard our great news! The pregnancy test was positive! I was really pregnant! It was the greatest moment of my life! We laughed and cried and hugged and went nuts! Augie went nuts with us. We called the nurse back and she said my estrogen levels were a little low and I needed to take some meds to boost them. I asked her if that was a big cause for concern, like maybe we shouldn't celebrate. She said it was a red flag, but that I was definitely pregnant and we could celebrate. She suggested waiting to tell people until we saw the heartbeat at 7 weeks, but again, I was definitely pregnant! Once I was done talking to the nurse, I called my mom and dad who were driving somewhere...I don't remember where. When my mom answered, I said, "Guess when my due date is?" And she responded, crying, "January 28!" She was exactly right! How did she know? Stay tuned to a future posting to find out!

The Results of Faith, Hope, and Perseverance: Devastation
For those of you who can't do math or don't know how many months it takes for a baby to grow, I'll fill you in. When we went for the pregnancy test on February 17, it came back negative. The small collection of cells that was our child did not survive in my uterus. We were devastated. Devastated doesn't even begin to describe it. We really thought we had climbed our biggest hurdle when there was a balance/normal embryo. We never really considered that it wouldn't live or in other words I would miscarry. On February 17, we had to travel to Iowa City for a blood test to determine if I was pregnant. Then, a few hours later we had to call in to a phone line to hear the results. I actualy forgot to mention the phone line earlier. Every day I had to call the phone line to hear how much drugs I should inject that day. On the day of the pregnancy test, the message on the phone line was not what I wanted to hear. The nurse who left the message gave us a number to call if we had any questions. Of course I had questions. I wanted to know why the test was negative. Unfortunately when we called, the nurse wasn't exactly very sensitive. This particular incident was the only time we were frustrated with the people at the clinic. The nurse made the comment "weren't you expecting it to be negative since it wasn't a blastocyst when it was transferred?" No. We had no idea. We were given every indication that the baby could and would live. After all that work, all the emotions, all the shots, all the trips to Iowa City, we came up with nothing. My initial reaction was that I was never going to be able to have my own child. And, at that point I was not comfortable with adoption, so I thought I was never going to have children. I currently have the most beautiful daughter in the world, yet I sit here crying as I type this entry because that moment sure ripped my heart out. After a few days we called the clinic again and asked if we could meet with our doctor to determine what, if anything, we should do next. We went in mid-March back to Iowa City and were escorted into an office. A short while later, 2 doctors came in. Not 1, not a nurse, 2 DOCTORS. Those doctors sat there and answered our questions, consoled us through our tears, and new our case forwards and backwards. They felt there was not anything in particular that caused the baby to not live in my uterus. They felt there was just as good of a chance to conceive if we wanted to try again. They weren't pushy, but they did give us hope. Never did I feel like a science experiment while we dealt with the in vitro clinic, but at that moment, I truly felt like the doctors wanted Justin and Julie Pegump to have a baby. They honestly wanted to help us. There was also that air of ego there; that "we can help these people have a baby". I am really grateful for that. Those doctors are awesome! (I'm leaving out their names just in case, but if you want to know, send me an email and ask!) When we finished with the doctors, a nurse came to talk with us about when we could try again. It was no question with us that we wanted to. I didn't care how many shots I had to get or how much blood they drew, I wanted to have a baby and this was our path to achieve that. We learned there was no reason we could not start in the next cycle, which was mid-April. We also learned there are 3 different types of in vitro cycles they perform. The first one we did was the standard, slowly give me more and more drugs to help the eggs grow. This time we were going to try a more aggressive approach where they hit me with more drugs in the beginning. The idea was to get more eggs to hopefully give us more than one balance/normal embryo. I can't even remember the third type of cycle because we didn't need it! The new cycle would start in just about a month.

The Results of Faith, Hope, and Perseverance: In Vitro
On September 25, 2008 we visited the Advanced Reproductive Technologies Clinic at the University of Iowa. It was an initial appointment and consult. It was an interesting and emotional day. We learned all about the in vitro procedure and how it would apply to us. We learned we were a very unique case. Basically, any assistive technology out there, we needed it to help us get pregnant. But, we were ready to try and to go through their process. We expected to start right away, but quickly learned they operate in 6 week cycles. We were unable to make it work to fit in the very next cycle so we had to wait until January of 2009 to start. At first I was devastated we had to wait longer. I am a basketball coach and January is right in the middle of basketball season. I thought our in vitro cycle would cause me to be unable to coach, but it didn't. And ultimately I knew having a child was much more important to me. There is more to this coaching story later.
In December I had to take one month's worth of birth control to get my body regulated. Then, in January the drugs and shots began. Without looking it up, I couldn't tell you the name of all the drugs I had, but I can tell you how it worked. It started with one shot in my leg every night for about a week. I gave myself the shot. We had to travel to Iowa City about every other day to give a blood sample and/or to have an ultrasound to see if I was producing eggs. Then, we added a shot in my stomach, which I gave to myself. A few days later, a shot in my back was added, which Justin had to give to me. He didn't want to at first because he didn't want to hurt me, but I told him we were partners through this whole process and there was no one else I wanted to give me the shot. So, three shots a night and several trips to Iowa City. Once they determined my eggs would be big enough to harvest, I got one free night with no shots and then one night with one shot to tell my body to release the eggs. I had to take that shot 36 hours before my egg retrieval surgery. After that the shots in my back continued to help my body think it was pregnant.
On February 2, 2009 we went to Iowa City for my egg retrieval surgery. It is a fairly simple process. I was put under and the doctors go in and retrieve the eggs from my ovaries. It was pretty painless for me and for once in my life I didn't throw up from the anesthesia! On that day, from all the drugs I took, they retrieved 14 eggs. The next medical step we needed is called ICSI, which is where they took 14 of Justin's sperm and tried to inject one into each of those 14 eggs. It only worked on 11 of the eggs. The other three were not mature enough to be fertilized. Then, they are basically put in an incubator for 3 days to let them grow. This whole process is measured in 6 days. Egg retrieval day is day 0. By day 3, our "children" were 3-5 cells. On day 3, only 8 of them were still going. If you read my previous post, remember our 1 in 8 chance. This is where it starts to show up. As you can see, our numbers dropped off as the days progressed. At this point, the genetic testing process begins.
The University of Iowa is a fabulous place for helping couples get pregnant. They have amazing doctors and nurses who we grew quite fond of in our experience there. But, even they couldn't do everything for us. They connected us with a company called Reprogenetics in New Jersey. Reprogenetics plays a part in the next step. On day 3, the embryologist took one cell from each of our 8 embryos and sent it off to New Jersey to Reprogenetics. Reprogenetics then injected dye into the cell that would attach itself to each end of the number 1 and number 10 chromosomes to be sure all the information was there. If they could see color on each end, it meant the child would be balanced like Justin or completely normal. They were unable to tell the difference between balanced and normal. At this point, we could have had every chromosome analyzed; found out boy or girl, if there were any other problems, etc. Of course that would have cost money and we wanted to try and be as "normal" as possible.
The most interesting thing about sending those cells to Reprogenetics is that the doctor at the University told us we had one superstar embryo and the other 7 weren't looking too good. In fact, by day 4, she said there were really only 5 that were still alive. She knew what she was doing because the results came back from Reprogenetics that the "superstar" was balanced or normal and the rest were not. It is important to point out that it was very possible none of the embryos would have been normal and we would not have had one to transfer on day 5. So, when they told us we had one, we were ecstatic and thought we had cleared our biggest hurdle!

In case you couldn't figure it out, the result of our faith, hope, and perseverance is our beautiful daughter, Aria. For Justin and I, having children was always something we knew we wanted; it just didn't happen when we wanted. We were married on August 7, 2004 when we were both 22 years old. Being the planner and numbers person I am, I immediately began predicting when our first of maybe 3 or 4 children would arrive. I wanted to be 25 when I had my first child because that is how old my mom was when I was born and I think our age difference is quite nice. 25 came and went. So did 26. And 27. At that point I wondered if I would be able to have a child before 30. I knew that 27 was not "old", but it felt like it. Let me back up. When we were unable to get pregnant at 25 and 26, we began seeking out the reason why.
Our investigation began with me and several tests to see if all was working properly. Everything came back normal and functioning. My doctor recommended I have laproscopic surgery to further investigate. We were a little skeptical that my doctor wanted to perform surgery on me before we even checked to see if there was a problem with Justin. So, we had tests run on Justin. The results were earth shattering. There was a possibility Justin could not reproduce. We learned this news in November of 2007, age 26.
The first step was for Justin to have corrective surgery to hopefully improve his chances. He had the surgery in January of 2008 and we had to wait 6 months to see if it worked. I was at basketball camp the day in June when Justin called to tell me the surgery had not worked. We were referred to the University of Iowa and one of the best specialists in the nation. Dr. Wold was very sympathetic and helpful with our situation. He gave us the facts and provided suggestions. He said he could redo the surgery because he felt it could be done better, but he thought we should have some genetic testing done first to make sure there was not another underlying cause of the infertility. This was in July of 2008. We thought the genetic testing made sense and went forward with it.
A funny side story...Justin had to give 8 vials of blood for the tests and he went on an empty stomach. Not a good combination. He passed out. As we continued through the process we had many blood tests and samples and that lady always remembered Justin.
Anyway, the results came back with something we really didn't understand. Justin has what is called a balanced translocation of his number 1 and number 10 chromosomes. To better help us understand we met with a children's genetics doctor at the University. Basically, when Justin was created, part of the information on his number 1 chromosome switched places with part of the information on his number 10 chromosome. No information was lost in the switch, which is why Justin is perfectly normal. (Although we like to tease that maybe it creates excess body hair or excess gas!) However, when Justin tries to reproduce a large problem is created. For those of you that don't know DNA, everyone has 2 of every chromosome 1-23 and then the X and Y. When making a baby, the male passes along one of every one of his chromosomes and the female passes along one of every one of hers. So with Justin, the problem occurs if he passes the number 1 chromosome that has part of number 10 with it and not the number 10 that has part of number 1 with it because then, the baby would be missing part of the information from the number 1 (or number 10) chromosome. It's pretty confusing, but we learned quickly.
The good news was that we could have a child, created by the two of us. The bad news was that happening naturally was virtually impossible. When passing along his chromosomes, there are 16 options for Justin with his condition. Out of those 16, 2 have a chance of being a normal baby. The other 14 would have great genetic defects and only have a 2% chance of becoming a living breathing child, with great defects that would likely mean a short life. Therefore, math whiz kids, we had a 1 in 8 chance of creating a balanced child like Justin or a completely normal child. 1 in 8 is pretty grim, but you also have to take in to account that is AFTER conception. We had slim chances of natural conception, too. So we needed help in creating our child.

Introduction
Inspired by a truly great and talented friend, I am going to attempt this digital world of blogging. Since January 29 there are many great things to share with those of you out there interested. Of course I know I had things to share before, just none as cute. My purpose is to share pictures, videos, and updates about Aria (and of course Justin, Augie, and I as well). I also hope to build my own reflection abilities and maybe influence someone out there in the process. I love reading others' blogs, so it's time I get my own going. Feel free to comment and encourage what you want to see and read!
